Dentist versus Boredom: A Journey of Professional Satisfaction

As a 1979 graduate of the University of Tennessee Dental School, after practicing dentistry for 17 years, I, like many colleagues, found the traditional dentistry that is taught in school was failing to meet the needs of my practice. Most dentistry seemed to be replacing failing restorations from previous treatments. What could keep the replacement dentistry from future failures? On a quest for professional satisfaction and to curb boredom in the field, I set out to answer this very question. As I began this journey in 1996, the theme of “less is better than more” emerged. I wanted to preserve more healthy tooth structure. It has been a long journey but here is the short story of how I arrived at the Alleman-Deliperi Center.

To effectively do less and better dentistry, a dentist must enter the world of bonding and be very good at it. The first stop in my journey for better bonding was air abrasion in 1996 with Crystal Mark. I needed to maximize my bond with a cleaned bondable surface and at the same time increase the effective surface area. With air abrasion and resin coating as taught by Dr. Raymond Bertolotti, I found my composite restoration sensitivity go to near zero. I was making progress towards my goal.

The second step in my journey was CEREC. To maximize bond strength, I knew I must minimize the bond stress. I entered the world of CEREC 2 in 1998. After all, CEREC is nothing more than a composite with a large macro particle sitting on top of a composite. When the tooth volume lost is significant, CEREC allowed me to minimize the bond stress and keep more healthy tooth structure. With CEREC and the near zero sensitivity, I found a tremendous drop in the need for post-opt endodontic treatment. Unlike traditional crowns, I also found that gingival inflammation was eliminated.

I am now at a third major point in my journey to achieve “less is better than more” dentistry. I was introduced to the Alleman-Deliperi Center by Dr. James Erpenbach in 2017. Here, I have learned new techniques to improve the bond strength and minimize the bond stress. All of these “new” techniques are supported by research. With this new information, I foresee that CEREC will become less significant. One contributing factor to this is that the Alleman-Deliperi Center teaches Stress Reduced Direct Composites (SRDC) which rivals the strength of natural tooth structure and allows dentists to preserve even more healthy tooth structure than CEREC. Another reason SRDC rivals and in my opinion is better than CEREC is because CEREC is not cheap. Your lab bill has simply changed to a fixed loan payment. CEREC is a developing technology requiring upgrades every few years and new trainings. My purchases have been significant including going from CEREC 2 to CEREC 3 (Red Cam) then to Blue Cam and Omni Cam along with 3 milling units and 1 rebuilds. I am now questioning if I will even get another CEREC when Sirona is requiring upgrading. It seems the return on investment has been greatly reduced.

This September I had the opportunity to attend the biomimetic course in Cagliari Italy. What a wonderful experience. I spent the week with a group of incredible dentists and associates who are leading the pack in biomimetic training. The combination of teachers, material, facility left nothing to be desired.
